Presurgery Checklist

Hospital Specific Requirements

UCSF Benioff Children's Hospital

  1. Pre-operative History & Physical (H&P): Your child's Pre-operative History & Physical must be scheduled within one month of their surgery. The H&P may be conducted by our surgeon or by your child's primary provider pediatrician. If this is done by your pediatrician, the H & P can be faxed to our office at (415) 476-2929.
  2. pedi-PREPARE: We offer families a pre-surgical appointment with pedi-PREPARE, a division of the Department of Pediatric Anesthesia. During this appointment, a nurse practitioner will obtain a medical history and answer your questions regarding anesthesia. They will also review fasting instructions. This appointment may be conducted by phone or in person, depending upon the recommendation of the surgeon.
  3. Child Life Specialist: In order to help prepare your child emotionally for surgery, you may meet with a Child Life Specialist (for more info on our Child Life Deptartment read Preparing Your Child for Surgery).
  4. Preoperative blood work: While usually unnecessary, any blood work required can usually be drawn at the lab of your choosing and the results faxed to our office in advance of the operation.

CPMC California Campus

  1. Pre-operative History & Physical (H&P): Please make an appointment for your child to have a preoperative history & physical (H & P) with his or her primary provider within one week of the operation. The H & P can be faxed to our office at (415) 476-2929.
  2. Pre-admission: Families are required to pre-register their child up to 2 weeks before the operation, in the pre-admission department on the first floor of the main hospital. Office hours are 8:00 AM to 4:30 PM Monday through Friday. The phone number to make an appointment is (415) 600-2500.
  3. Preoperative blood work: While usually unnecessary, any blood work required can be drawn at the time of the appointment.

Kaiser Permanente Medical Center, San Francisco

  1. Pre-operative History & Physical (H&P): Your child's Pre-operative History & Physical must be scheduled within one month of their surgery. The H&P may be conducted by our surgeon or by your child's primary provider pediatrician. If this is done by your pediatrician, the H & P can be faxed to our office at (415) 476-2929.

Insurance Approval

All elective operations require authorization from your insurance provider. Our office staff is experienced in obtaining authorization, however in rare cases, authorization from your insurance provider may be denied. If this occurs, the operation will be postponed until approval can be obtained.

This does not affect Kaiser patients cared for within a Kaiser facility.

Presurgical Eating & Drinking Guidelines

It is very important that you follow these guidelines. If these guidelines are not followed, your child's surgery will be delayed until the end of the day or re-scheduled. The fasting guidelines are:

  • After midnight: Stop non-clear liquids and solids (this includes any food and also hard candy or gum)
  • 6 hrs before arrival: Stop infant formula
  • 4 hrs before arrival: Stop breastmilk
  • 2 hrs before arrival: Stop clear liquids (water, clear apple juice)

Presurgical Medications Guidelines

If your child is taking Aspirin, Ibuprofen (Advil) or other anticoagulants (blood thinners), please inform our office at least 7 days in advance of the operation to avoid delays in scheduling.
