Depending on your insurance, we may require a referral from your pediatrician and insurance approval prior to scheduling your appointment. Interpreters are available upon request for all non-English speaking patients.

What to bring to your appointment
It is important that you bring the following with you when you come to see us for your child's appointment:

A list of your child's current medications and the dosages your child is taking. You may bring the bottles or containers with you to your first appointment.

A CD of all your medical tests and medical imaging studies that were done at an outside facility (X-Ray, Ultrasound, CT scan, MRI with the dictated report, etc.).

The name, address and telephone number of your physician who referred your child and your primary care provider if they are different.

Your child's health insurance card and Medicare/Medi-Cal card (if applicable).

Any additional documents you were asked to bring with you.
Scheduling your surgery
If you and the surgeon have decided to proceed with surgery, you will need to speak with our surgery scheduler, Brian Forman at (415) 476-2539. Brian will provide you with the location, date and time for the surgery as well as pre-operative fasting instructions.
